Appalling level of care, zero communication,

Having been referred to this team for the second time, I find their communication and care extremely poor. Three weeks have passed since a meeting to formulate my care plan I am still waiting to be told the outcome. My G.P, having contacted them today, was told they couldn’t read the minutes of the meeting and I would be contacted today. Nothing has happened. This is a completely unacceptable situation which does not help anyone who has anxiety or depression, let alone suicidal thoughts.
