Poor hospital discharge prevented medicines to be taken

Patient was taken into hospital on a Saturday and discharged on the same day, medication that went in with the patient were not returned and this included antibiotics. Patient missed doses of their course as out of hours could not support until the following day, of which time the patient had missed several doses.
