Appalling

I work in the NHS and have regularly liaised with this service in recent years. In a nutshell they do not provide adequate support for patients at any stage of the process. It is such a minefield of problems, I don't know where to start to be honest. We regularly see patients who are not offered ongoing support in the community via the care coordinator system. This continues in spite of regular letters specifically requesting this. These letters are ignored. The impression the care coordinator is that they are poorly skilled for the task given to them. The system needs a massive overhaul. I have been in contact with my MP, I understand that Jeremy Hunt has had a letter about it.
