Wrong info: from Guildhall Surgery

I was told to fill in acceptance form and leave at Guildhall Surgery, Clare. I had jab 19th Jan. No problem. My wife given a date for jab by Guildhall Surgery a few days later but, after some confusion, was told to fill in acceptance form and take it with her to Lavenham jab centre 24th Jan. I am now concerned. Is my acceptance form still at Clare? Should it have been sent to Lavenham? I had no problem at Lavenham. Staff were friendly, professional, A1.
