Lack of communication and assistance

I'm writing this on behalf of my son. He has ADHD and has medication for that, unfortunately there is a lack of the medication. This is the 2nd time, I was advised to speak to the GP to ask for the liquid for of his medication (via the ADHD team) which I did, the GP surgery had received a letter explaining that and I also have a copy of that letter. So when I went to follow up with the new prescription, I was told that the pharmacy team were declining to issue a new prescription because it wasn't on my son's notes (letter was dated 31/7) and I had to go through the ADHD Team who had already confirmed that it should be sorted my the GP team. We've had many issues with the surgery and prescriptions.
