No care plan made. Promises made and not kept

No care plan made in 3 months of meetings in my home. I was patient. Promises made to assist me with some forms, not done in 3 months. I rang their out of hours service to be told once no record.. then they did have a record of it. I rang teir day time service with a long message.. to a person.. who interacted with me about my meds.. told no such record of call existed. I asked for this to be looked into please and I wanted to suspend visits until this was answered. I was rudely dealt with by an overfirm manager who took my questions as attacks. nothing of the sort .. I wanted to understand what was happening. nothing more. despite me asking to merely suspend visits I got a letter saying.. 'sorry you have decided you do not need our service' . and complaints leaflet. I at NO TIME said I was suspending this service. I spoke to another worker there who told me to 'calm down'.. I wasn't anything other than calm.. I simply asked why she was putting me bac on waiting list after she said. 'you have been offered two appts and haven't kept them'. I explained. the first one arrived on a Satuday lunch time.. I had a commitment, long standing, and other medical commitment. on the da in question.. two days later on the Monday. I rang in on the Monday morning to cancel this. the second one was apparently phoned to my answer phone..I explained I got an incoherent message from an unknown source as it didn't come out clearly enough to know what it was about or from whom. She repeated to me four times. as you failed to keep the two appts given to you I am referring this back to management. Where is the sense or fairness in this. I did say. I am perfectly calm, thank you, I a asking questions.. a totally different thing to being non calm.
