Notes not read, treatment not carried out

I have in grown eye lashes so the consultant removed these at the clinic prior to having my cataract operation. Before having the operation I needed to have where the eye lashes were removed cauterized, but on the second visit to the clinic for the cauterization they removed more lashes rather than cauterizing. When I went for my operation (cataract) the surgeon did do the operation but said I would urgently need to be re referred for the cauterization as it would damage the new lens if left undone. The second doctor didn't read my notes properly so carried out the wrong procedure.
