Communication between departments is bad

Husband went in for a perforated bowel. On warfarin. I couldn't take him off that in time to go in for the operation as was an emergency. During surgery they caught his spleen and caused major bleed and so they had to remove it. Now permanently on antibiotics. Nursing team amazing. Communication between departments is shocking. In April 2016 he was in for urine retention and they did heart scan. Took 2 weeks to get results. In the meantime we saw doctor - doctor didn't know it had been done
