Delayed discharge/transfer of care

I am frustrated that there is a delayed transfer of care. People are in hospital and staying in the hospital for far too long (in relation to mental health beds) as there is no beds or care available to them to go to once fit. The hospital is a good service but the ongoing need is not catered for.
