Medications conflict with conditions

The medication issued can sometimes conflict with other medical conditions. It is only by doing my own research that I inform the GP that my partner should not have it. For Example: A tablet linked with his heart condition was doubled, then doubled again. It conflicted with my partners Dementia really badly. The GP did not seem to know anything about it, it is like I am educating them.
