Inappropriate discharges

A gentleman who had dementia and went into hospital from his residential care home with pneumonia was treated poorly. His family were told that he was at end of life and were very distressed at the news. Without any handover or information given to the care home, the gentleman was brought to the door of the Care home by ambulance transport on a stretcher and in a very poor state. The gentleman could not be accepted by the home as the residential care home staff could not safely care for him in the condition he was in due to his new diagnosis. The Care home never received any communication from the hospital.
