Poor communication with the hospital

After being discharged from hospital I had to chase the surgery to get the medication I needed to be on. Apparently the surgery hadn't received anything from the hospital so were unaware of the situation. I'm also not happy that you don't get to see a doctor as standard although the telephone calls to the practice more recently have sorted some medication problems out.
