Very poor, discharge letter incorrect

Discharge letter on 17/06 states that a left side fracture was sustained, when in fact the fracture injury was on the right side.
Discharge letter on 24/06 states that there was a right side fracture on pg1, on pg2 it states that the patient had a new fall. In fact they were admitted for breathlessness.

It is not a good system that the doctors visit in the morning and the family can only visit in the afternoon receiving little or no information from the ward nurses.
There is a total lack of communication as to the situation regarding the despatch is appalling.
Firstly we were not even being informed that our father had left the ward and was waiting for an ambulance for over 2 hours in hospital pyjamas, not even in his own clothes. Finally arriving over three hours later, cold, tired, hungry and needing to use the toilet.
Neither were we allowed to travel with him in the ambulance.
Something has to be changed, this is outrageous.
There is a total lack of communication as to the situation regarding the despatch is appalling.

The total time that I was seen and diagnosed with vertigo was within the free parking limit on the car parking machine.

Came to A&E with pains in chest and told to have blood tests which took ages.
Not as good as usual, been in 12 times over the last two years and it's usually better than this, only 1 doctor available.

This visit was a GP referral, I have waited 3.5 hours for meds to arrive.
Not even offered a cup of tea.
So long as parking is free, but the charges need reviewing - charged for disabled and it's too high.

Ambulance arrived within moments of the phone call from the doctor.
Suffering with a bad chest which is more agitated with the 3 hour wait to be seen.
